[Institutional Strategic Goals Supported] Only the strategic goal (s) from the current college strategic plan to directly relate to the program/offices mission and responsibilities – same as 1-6

[Unit/Program Mission Statement] As approved by President through appropriate standing committee (Curriculum Committee for instructional programs; Student Services Committee for Student Services Programs; and Planning and Resources Committee for Administrative and other programs) – same as 1-7

Administrative and student services - First present a philosophical statement related to your units/program/office followed by a listing of the services you provided)

[Unit/Program Outcomes/Objectives]

SMARTer (specific, measurable, achievable, realistic, Timebound) should be used.

All outcomes/objectives should include specific criteria to determine success.  This could be the percent improvement goal (say 75%) on a student satisfaction survey or what per cent students in course will meet the SLOs.  At the institution and campus levels this should reflect critical quality indicators and key results (graduate and retention rates, employer satisfaction with graduates, successful external transfers, etc.)
