in the Description of Change the last revision shown on the Standard Drawing or Special Provision you intend to use. If the change involves an item that requires a new Special Provision, write the new Special and include it as an attachment to the proposed change.
NOTE C: If there is not sufficient space on the Change Order face to write a clear Description of Change and Reason for Change, use an attachment. Indicate Job Number and Change Order Number on all attachments so that the attachments will not lose their identity in case they become separated from other parts of the Change Order.
Item 9 - The specification item as shown in the contract, 'SP' if a Job Special
Provision only is applicable, the Standard Specifications
or the appropriate Specification item as shown in

Item 10 - Descriptive name of the item. For items that are a part of the original contract this MUST BE the same as shown in the contract. For items in the contract which have a price change, this description may be the contract name followed by the change order number, e.g., "Borrow (C.O. #3)". This entire phrase is then the Item name.
Item 11 - Unit of measure as shown in the contract or as agreed upon.
Item 12 - Contract unit price or price agreed upon. If this item is not a part of the original contract, this agreed upon unit price shall be documented as discussed in Paragraph 104.01, and will require an executed Supplemental
Agreement prior to being included for payment on an Supplemental Agreement will be prepared by the Construction
Items 13 through 18 - The total plan quantity should not be shown in the "Present Quantity" column unless the change represents a revision of the quantity throughout the job, such as changing the cubic yards per station of base course material over the entire length of the job. The change should only reflect the quantities involved within the limits designated. For example, if the change involves altering the size of a box culvert, the total plan quantity for Concrete, Reinforcing Steel and Structural Excavation would not be shown. Under "Present," the plan quantities for the original structure at that particular location should be shown. Under "Revised" the quantities for the proposed structure should be entered.
If the change involves installation of an additional structure, the present quantity for each item should be zero and the revised quantity shall reflect the overrun.
Earthwork Change Orders very seldom start or terminate at a fixed balance point and it is difficult to isolate the quantities between the limits of the change; however, efforts must be made to isolate these quantities as accurately as possible.
