"Approved Trainee Record" form (Figure 107-10) is to be used for this record. This form is set up for a trainee when the Resident Engineer receives a completed Registration Card from the Contractor. Information relating to the trainee from the Contractor's weekly payroll will be entered on this form as the payrolls are received. The Resident Engineer shall determine that the information on each current payroll corresponds with information
previously submitted (i.e., SSN, training classification, address, etc.)
The "Approved Trainee Record" is used to verify the hours worked by each trainee shown on the Training Program Estimate, which is prepared by the Resident Engineer.
It is also a responsibility of the Resident Engineer to see that each trainee is being paid at least the minimum wage required by the training program and that each trainee is being paid according to the stage of training which has been achieved.
(e) Trainee Termination. The "Trainee Termination Form” (Figure 107-11) shall be completed by the Contractor on each trainee that is terminated from the training program or changes training classifications. When this form is received by the Resident Engineer, immediate verification shall be made of the information submitted. Any discrepancies should be discussed with the Contractor's EEO Officer and the necessary corrections made.
Before a trainee can be terminated in one training classification and enrolled in a new classification, the Contractor should request approval from the Resident Engineer to enroll the trainee in the new classification. The Resident Engineer should review the request, before approval, in order to ensure that the trainee will be able to obtain the required hours to graduate. If the new training classification is listed as critical on the OTPSP, the Contractor will be entitled to reimbursement of $2.00 per hour for each hour trained.
The Contractor must complete and submit a "Trainee Termination Form" terminating the trainee from the previous classification and a Registration Card enrolling the trainee in the new classification. The Resident Engineer will be required to complete and maintain a new "Approved Trainee Record" for this trainee in the new classification.
