The NHS Confederation has been working in partnership with 20 organisations, representing patients, managers and professionals, to make the case for a better deal for people with long-term conditions. Other organisations included Diabetes UK, the Long Term Medical Conditions Alliance and the Stroke Association.
8 September saw the launch of the partnership's manifesto '17 million reasons', which sets out a shared vision for people living with long-term conditions and outlines a series of practical proposals to transform the lives of one third of the population.
These include that all such patients should have an externally audited care plan, more rapid access to diagnosis amd more self-referral.
