Please follow the below guidelines when requesting modification changes to your contract:
1. EFFECTIVE May 1, 2008, all modification requests not submitted through eMod must be submitted to the Central Intake Desk (CID). Modifications should be submitted to the CID one of these three ways –

Modification requests must be accompanied by a completed cover page checklist (see next page). Effective June 1, 2008 – All modification requests submitted without a completed cover page checklist will be returned without consideration. Other than the cover page checklist, there is no change in the documentation required for the various types of modifications. You should continue to submit the same modification package and /or documentation to support you modification request that you have in the past.
