Population Diagnosis and Priorities
The public health nurse analyzes the assessment data to determine the population diagnoses and priorities.
Analyze the collected assessment data to determine if there is an identified, actual or potential problem.
Verify the medical diagnosis and determine the priority of action:
Review Section/page D1-D2 of the Public Health Nursing Practice Manual for priority per Acute Communicable Disease Control (ACDC) or determine the priority of action in consultation with the PHNS as needed. Document priority selected.
Consider the client’s/contacts’ need for nursing interventions based on the medical diagnosis.
Consider the client’s/contacts’ need for nursing intervention to promote health, facilitate well being, foster healing, alleviate suffering, and improve quality of life.
The public health nurse identifies expected outcomes for a plan that is based on population diagnoses and priorities.
Prevent the spread of communicable diseases within
families, communities, health facilities, or other sites.
