Standard of Practice
The public health nurse evaluates the health status of the population (all affected by the case).
The PHN examines the effectiveness of the interventions, including the need for modification of the interventions in relation to outcomes.
Evaluate the effectiveness of the interventions on the health of the client/contact(s); e.g. document client understands the disease process and prevention of transmission:
Determine and document client/contact(s) adherence:
Medical evaluation is obtained.
Treatment is completed.
Determine action for non-adherent client/contact(s):
Consult with PHNS.
Document in Casewatch®:
Disposition and close case within 14 days of receipt of
referral or within timeframe agreed upon in consultation with PHNS.
Document in the NPMS:
File a copy of the PHN Assessment per the PHN Assessment Form instructions.
Evaluate client satisfaction:
Give client satisfaction form to the client for completion and submission in a pre-addressed, stamped envelope.
