Provides developmental screening, assessment, health education, and case management services to families with children birth to 5 years of age. Provides low-cost immunizations for those who qualify. Immunizations include: Polio, Tetanus, Diphtheria, Pertussis, Measles, Mumps, Rubella, Hib, Meningitis, Prevnar, Varicella, and Hepatitis A and B vaccines. Provides a free resource kit for parents and caregivers of children birth to 5 years of age.
