PROGRAM DESCRIPTION Provides information on affordable housing, home rehabilitation loans, fair housing issues, and first-time homebuyer opportunities.

Information, assistance, and referral services; health insurance counseling and advocacy program; Community–based Care Transitions Program (CCTP), case management for age 18+ disabled persons or age 65+ (MediCal eligible persons with no share costs), to prevent premature institutionalization. Administer funds for Older Americans Act to senior organizations. Assists victims of sexual assault, domestic violence, senior crime prevention, victim compensation, and crimes against the disabled. Provides orientations to the criminal justice system, court support for Temporary Restraining Orders, and a service referral program. Investigates reports of elder or dependent adult abuse or neglect, including self-neglect. Based on an assessment of their findings, staff develops a plan to preserve the rights, interest, safety, and welfare of the elderly or disabled adult client. In-Home Support Services provides domestic chores and non-medical personal care to low- income, MediCal eligible, aged, blind, or disabled persons in their homes. To be eligible for services, persons must be receiving SSI/SSD benefits or meet the program’s financial need requirement.
