4.

Customer Obligations

The Customer undertakes to immediately notify CMI of any change relating to the Account information provided herein by written notice no later than 30 calendar days prior to the due date of the next pre-authorized debit payment.

The Customer represents, warrants and guarantees that all of the Account information provided herein is accurate and that all persons whose signatures are required on the Account have signed this PAD Agreement below.

The Customer acknowledges receipt of a signed copy of this PAD Agreement.

The Customer fully understands the terms and conditions of this PAD Agreement and hereby agrees to participate in this pre-authorized debit arrangement.

Option 1 We enclose a sample “VOID” cheque.

Please note that the VOID cheque or Bank Generated Account Validation Form must be provided in addition to, and not in place of, the Pre-Authorized Debit Form.
