7.17.3

his identifying number which appears in the Relevant Register;

7.17.4

the date:

(a)

he ceased to be employed or engaged in its practice,

(b)

his functions changed so as no longer to include prescribing, or

(c)

on which he was removed or suspended from the Relevant Register.

Signing of Documents19  

7.18

In addition to any other requirements relating to such documents whether in the Contract or otherwise, the Contractor shall ensure that the documents specified in Clause 7.19 include:

7.18.1

the clinical profession of the Health Care Professional who signed the document; and

7.18.2

the name of the Contractor.

7.19

The documents referred to in Clause 7.18 are:

7.19.1

certificates issued in accordance with Clause 10.31, unless regulations relating to particular certificates provide otherwise;

7.19.2

Prescription Forms; and

7.19.3

any other clinical documents.

Appraisal and Assessment20

7.20

The Contractor shall ensure that any medical practitioner performing services under the Contract:

7.20.1

participates in an appropriate appraisal system; and

7.20.2

co-operates with an assessment by the NCAA when requested to do so by the PCT.

19 Clauses 7.18 and 7.19 are required by APMS Direction 5(1)(g).

20 Clause 7.20 is required by APMS Direction 7.  However, APMS Direction 2 would also permit the parties, if they wished, to specify in the contract what would constitute an appropriate appraisal system.
