The City needs your help. It is conducting an historic health survey – the New York City Health and Nutrition Examination Survey. Several households in the [blank] area have been selected at random for potential participation.
The survey will identify how many New Yorkers really have diabetes, have high cholesterol, or are suffering from depression, as well as several other health conditions. The NYC Department of Health and Mental Hygiene pays participants $100, as a way of saying 'thank you' for their time, and provides each participant with their health results and any necessary referrals. All information provided is strictly confidential. In fact the survey has a Certificate of Confidentiality from the National Institute of Health, the highest level of confidentiality available.
Participation is easy - and those who are selected to participate will be asked to come to a health center, either in Queens or Manhattan, for a brief physical exam and interview. Car service to these locations can be provided.
