Package of Prevention Scholarship Terms and Conditions of Participation In order to be eligible to participate and be considered for Package of Prevention’s Beryl V. Smith Scholarship, the application must be accompanied by a signed and dated Acknowledgement.
Terms and Conditions of Participation: 1. No purchase necessary. Open to all legal residents of the state of California who have lost a parent or legal guardian to cancer or have a parent or legal guardian who is a cancer survivor. To apply for a Beryl V. Smith Scholarship, please complete the scholarship application provided by Package of Prevention.
Send the Application Materials – Personal essay; signed Acknowledgement; a copy of the cancer patient’s death certificate (with cancer identified as the cause of death) or doctor verification letter; and proof of acceptance by the selected educational institution – to the following address:

Package of Prevention directors, employees and their immediate families may not participate in the scholarship program. Applicants submitting the application materials must be acting on behalf of themselves, and if necessary, with the consent of a legal guardian. Applicants must provide current contact information – Name, address, telephone number(s), e-mail address, etc. – in order to be notified regarding their scholarship application.
All Beryl V. Smith Scholarships are valid for up to the total amount of the award. The scholarship will be valid for one academic year following receipt of the award. Scholarships cannot be carried over to subsequent academic years. Applications for each Beryl V. Smith Scholarship award period will be accepted during the period beginning December 1, 2009 and ending April 24, 2010.
Applicants who have been selected to receive a Beryl V. Smith Scholarship will be notified by August 1, 2010 via first-class mail. Odds of being selected for a Beryl V. Smith Scholarship will depend on the number of valid applications received. Names of scholarship recipient(s) will be posted on the Package of Prevention website within 60 days of the application submission deadline. Winners may be asked to submit a brief biography and /or personal photograph that will be posted on the Package of Prevention website.
Only one application per applicant. Except where expressly prohibited by law, participation in the Package of Prevention Scholarship Program represents applicant’s express acknowledgement and agreement that Package of Prevention, its parent, subsidiary, or affiliates, may print, reprint, publish, advertise, promote, and/or otherwise exploit, in any medium, the application materials (excluding the parent/guardian’s death certificate and doctor acknowledgement form) submitted pursuant to this scholarship program, without any payment or consideration to the applicant or anyone authorizing applicant’s use of any of the application materials. In accordance with the foregoing, all application materials (excluding the parent/guardian’s death certificate and doctor acknowledgement form) submitted to Package of Prevention can be used at the discretion of the organization for all future marketing, promotional, and royalty-free use.
All application materials received within the application submission period identified in Paragraph 3 above will be reviewed before winners are posted on the Package of Prevention website.
Package of Prevention will consider only application materials that are complete and properly submitted. No applicant will be denied a Beryl V. Smith Scholarship based upon gender, race,
